
Hey GlowGetter. If you’re in your late 30s or 40s and it feels like your body and mind are suddenly running on a completely different operating system, I want you to hear this clearly: you are not losing your mind, and it is not “just stress.”
Hot flashes and missed periods get all the attention when people talk about perimenopause, but the truth is much bigger than that. Perimenopause is a full-body hormonal transition, and it can touch nearly every system you have. Estrogen receptors exist everywhere, from your brain all the way down to your toes. When hormone levels start swinging wildly instead of gradually declining, the symptoms that show up can feel completely unrelated to anything reproductive at all.
This is one of the most frustrating parts of this whole transition, and it deserves an honest explanation.
Most primary care doctors, and even a lot of gynecologists, receive very little formal training on the menopausal transition during their medical education. On top of that, estrogen doesn’t decline in a smooth, predictable line during perimenopause. It fluctuates wildly, sometimes within the same day. That makes a single blood test, like an FSH or estrogen panel, notoriously unreliable for confirming what’s actually happening.
So when a woman walks into an appointment describing anxiety, joint pain, insomnia, and fatigue all at once, it becomes very easy for a provider to label it burnout, generalized anxiety, or lifestyle stress instead of connecting the dots back to hormones. Understanding the full range of possible symptoms is one of the most powerful tools you have to advocate for yourself and push for the care you actually need.
Let’s start here, because this category tends to surprise people the most.
Brain fog shows up as memory lapses, trouble finding the right word mid-sentence, or difficulty focusing on something that used to come easily. Irritability, sometimes called perimenopausal rage, can feel like sudden, intense anger that seems way out of proportion to whatever triggered it.
Anxiety and panic can spike seemingly out of nowhere, sometimes even waking you up at night.
Mood swings happen because of fluctuating neurosteroids, not because anything in your life has actually changed.
And depression or low motivation can settle in, tied directly to dropping estrogen levels rather than any external circumstance.
Physical symptoms are often where the confusion really sets in, because they don’t obviously connect back to hormones at first glance.
Joint pain and stiffness happen because estrogen helps reduce inflammation, so as it drops, joints can ache, especially first thing in the morning.
Unexplained weight gain is tied to shifts in metabolism and fat redistribution, particularly around the abdomen.
Muscle tension and aches can show up as persistent tightness, especially through the neck and shoulders.
Fatigue during this transition tends to feel like a deep exhaustion that isn’t fixed by a full night of sleep.
And some women experience electric shock sensations, a quick, sharp feeling under the skin that often shows up right before a hot flash.
This category surprises a lot of women too, because it feels so disconnected from anything hormonal.
Itchy skin, sometimes called formication, can feel like insects crawling on or under the skin, caused by collagen loss and drying.
Hair thinning or loss on the scalp can happen at the same time facial hair increases, both driven by shifting hormone ratios.
Brittle nails start cracking, peeling, or breaking more easily than before.
Dry skin and loss of elasticity are linked to reduced collagen production.
And adult acne can show up again, often concentrated along the jawline.
This is the category most people already associate with “the change,” but it goes deeper than most expect.
Hot flashes bring sudden waves of intense heat across the upper body and face.
Night sweats are the nighttime version, sometimes drenching enough to wake you up.
Insomnia can mean trouble falling asleep, trouble staying asleep, or waking up wide awake at 3 a.m. for no clear reason.
And chills can follow right behind a hot flash, as the body overcorrects.
Yes, and this category tends to be one of the most overlooked.
Tinnitus, or ringing, buzzing, or humming in the ears, can show up during this transition.
Hormonal headaches and migraines can be triggered by sudden estrogen dips.
Dry eyes and vision changes happen due to reduced lubrication, leading to irritation or blurriness.
Burning mouth syndrome causes a burning sensation on the tongue, gums, or lips.
And bleeding gums and other dental issues can increase due to heightened sensitivity.
It absolutely can, and this is another area where symptoms often get mistaken for something else entirely.
Bloating and digestive changes can bring increased gas, constipation, or generally sluggish digestion.
New sensitivities and allergies can develop, sometimes to foods, skin products, or environmental triggers that never bothered you before.
And changes in body odor can happen due to shifts in the skin’s microbiome and how the body regulates temperature.
This category is deeply personal, and it deserves to be talked about just as openly as everything else on this list.
Vaginal dryness and discomfort, sometimes referred to as Genitourinary Syndrome of Menopause, comes from thinning and drying of vaginal tissue.
Low libido reflects decreased desire or arousal.
Painful intercourse can result from the same loss of tissue elasticity and moisture.
Urinary incontinence or urgency can show up as frequent urination, leaks when coughing or sneezing, or sudden urges that are hard to control.
Recurrent UTIs can become more common due to changes in urinary tract pH.
And irregular periods, cycles that run shorter, longer, heavier, lighter, or skip entirely, are often one of the earliest and most recognized signs of this whole transition.
If you read through that list and recognized several symptoms in your own life, here’s where to go next.
Start tracking your symptoms for 30 to 60 days using a journal or a tracking app. Note when they show up in relation to your menstrual cycle, since patterns matter here. When you sit down with a provider, bring that log with you and ask directly about evidence-based management options, including Menopausal Hormone Therapy, non-hormonal prescription options, and targeted lifestyle changes. Walking in with real, tracked data changes the entire conversation, and it makes it much harder for a provider to wave your experience away as generalized stress.
If there’s one thing I want you to walk away with, it’s this. What you’re feeling has a name, it has a biological explanation, and it is real. Over 30 recognized symptoms exist for a reason, and knowing them gives you language, and language gives you power, especially in a doctor’s office where five minutes can decide whether you leave with real answers or another vague diagnosis of stress.
You deserve to be heard, and you deserve a provider who takes the whole picture seriously, not just the symptoms that happen to be the most talked about.

Perimenopause typically begins between ages 40 and 44, though it can start in a woman’s mid-to-late 30s. It usually lasts anywhere from 4 to 8 years before full menopause, defined as 12 consecutive months without a period, is reached.
No single blood test can reliably confirm perimenopause, since hormone levels fluctuate significantly hour to hour during this phase. Diagnosis is based primarily on age, symptoms, and menstrual cycle patterns rather than a single lab result.
Perimenopause is the transitional phase leading up to menopause, marked by fluctuating hormones and irregular cycles. Menopause itself is a single point in time, defined as 12 consecutive months without a menstrual cycle.
Many perimenopause symptoms, like brain fog, joint pain, or anxiety, don’t obviously connect back to hormonal changes, and limited medical training on the menopausal transition means these symptoms are often attributed to stress or aging instead of being properly identified.

✨ Separate age from identity. "I`m 48" is a fact; "I`m past my prime" is a belief. Treat them differently.
My mind was the first thing I had to rebuild. Before the debt payoff, before the 190+ lbs, before the new license and the new job, I had to change the story I was telling myself about who I still got to become. Save this for the days your mindset needs a rebuild too.
